OTHER SIDE EFFECTS

Some children and adolescents show a rebound effect about twenty to thirty minutes after the last dose wears off. For those on three doses a day, this occurs at about 8:30 to 9:00 p.m. For half an hour or more they may become very hyperactive and talk constantly or may be excitable or impulsive. If this side effect becomes a concern, the last dose may be reduced. This might make a difference. Another approach, if the medication does not cause sleep problems, is to give a fourth dose at 8:00 p.m. so that the medication does not wear off until the person is asleep.

There are other very uncommon side effects. Some children on Ritalin or one of the other stimulant medications become explosive, with a limited ability to control their anger. Some may become depressed or very anxious. Others might become very obsessive (with intrusive thoughts) or compulsive (needing to do certain behaviors or patterns of behavior). With each of these side effects, if the medication is stopped the behaviors return to normal either immediately or within a few days. If this person has one or more of the regulatory disorders, it may be that the stimulant caused an exacerbation of the disorder; thus the behaviors. As will be discussed in Chapter 16, it may be necessary to treat the regulatory disorder first and then return to trying a stimulant medication.
